GENERAL SUMMARY/ OVERVIEW STATEMENT: .

Provide patient care according to the philosophy and standards of Partners Continuing Care Associates. Execute and evaluate treatment techniques within the scope of Nurse Practitioner (NP) practice to assess and manage the program population; demonstrate advanced problem solving skills in all aspects of medical and nursing assessment, care and treatment. Use appropriate clinical skills to integrate and implement the nursing process with the plan of care. Be an integral part of interdisciplinary team working in Oncology unit. Function as a clinical resource for the patient, consistently providing direction and support to ensure a focus in the team goals and patient driven outcomes. Function within role expectations, delegate duties to other caregivers as required.

PRINCIPAL DUTIES AND RESPONSIBILITIES:


· Holistic, comprehensive patient care is provided in alignment with medical management team and in accordance with the nursing process and practice standards.

· Complete medical database is assessed including health history and physical examination upon admission of selected patients.

· Medical conditions are diagnosed, medical consultation is planned and ordered, and interventions are based on assessment and protocols under the direction a supervising physician or designee.

· Patient’s response to disease process and therapeutic interventions is evaluated and plans revised to achieve optimal patient outcomes.

· Plan of care is developed and implemented that reflects the needs and goals of the patient.

· Documentation includes all assessment data, orders, interventions, results and evaluations in the patient medical record per established standards.

· Information, education and support are provided to patients and families, including them in care and discharge planning.

· Quality improvement programs are participated in to achieve established goals.

· Nursing care activities are coordinated in collaboration with other care team members to facilitate achievement of patient goals.

· Ancillary nursing staff are coordinated and supported to achieve established performance goals.

· Nursing assessment is completed accurately and timely in accordance with practice standards.

· Medications are administered safely and according to professional standards of practice and nursing policies and procedures.

· Academic roles and research are supported to achieve established goals.

· Additional department, organization, or network activities are completed per established objectives.

SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:

Demonstrated competence in specialized area.

Experience with oncology preferred.

Computer proficiency required

High level of service delivery with ability to work with diverse populations

Ability to build relationships and collaborate effectively with a diverse population. Listen, understand and respond to concerns of patients and families, while displaying compassion and advocacy for patient needs.

Maintain variable work schedule to meet department needs (evenings, holidays, weekends and travel).

Ability to work independently, be self-directed and contribute as a member of the team.

LICENSES, CERTIFICATIONS, and/or REGISTRATIONS (if applicable):

Current licensure in the Commonwealth of Massachusetts as a registered nurse in the expanded role required.

EDUCATION:

Master’s Degree in Nursing required.

Current certification in ANP/FNP.

ACLS preferred.
