Description

As an Advanced Practice Provider, you'll play an integral role in our inpatient and ambulatory care teams by providing high-quality, cost-effective care to patients.

We are seeking a Nurse Practitioner or Physician Assistant to:

Provide comprehensive care to both well and ill adult patients in the outpatient and inpatient settings.

Perform independent clinic visits for returning advanced heart failure and transplant cardiology patients.

Be an active member of inpatient teams by participating in daily rounds and collaborating with other professionals to formulate plans of care.

Participate in multidisciplinary weekly rounds to make decisions for patients needing advanced care options or palliative care support.

Education:
Minimum –
If Nurse Practitioner: Master’s or Doctorate’s degree from accredited Nursing Program.

If Physician Assistant: Bachelor’s or Master’s degree from accredited PA Program.
Preferred – Master’s degree or higher.

Work Experience:
Preferred – Two (2) years working as an NP or PA with a cardiovascular patient population.

Licenses and Certifications:
Minimum -
If Nurse Practitioner: Active RN and APNP license in State of Wisconsin AND active board certification as a NP. These include the following: Adult, Adult/Gerontology Acute Care, or Adult/Gerontology Primary Care.

If Physician Assistant: Active PA license in State of Wisconsin AND active PA board certification.

All Nurse Practitioner and Physician Assistant applicants must have: Active DEA license or ability to apply for such license prior to or upon hire. CPR certification and advanced life support certifications if relevant to practice area (ACLS, PALS, NRP).

Physical Requirements:
Light: Ability to lift up to 20 pounds maximum with frequent lifting and/or carrying of objects weighing up to 10 pounds.

Work Schedule:
100% FTE, 10 hour days 3 to 5 days a week (8 days in a 2 week period) inpatient and outpatient rotations. No call, possible weekend rotation available, about 7:30 am to 6:00 pm.
