As an Advanced Practice Provider, you'll play an integral role in our care teams by providing high-quality, cost-effective care to patients.

We are seeking a Nurse Practitioner or Physician Assistant:
Partner with primary care providers, this role manages UW Health patients staying in long term care facilities.
A very independent role, you will located at Dane County long term care facilities working directly with patients and families.
This dedicated team provides outstanding care to our patients

Education:
If Nurse Practitioner:
Master’s or Doctorate’s degree from accredited Nursing Program

If Physician Assistant:
Bachelor’s or Master’s degree from accredited PA Program

Work Experience:
Preferred – Two years of relevant clinical experience

Licenses and Certifications:
Minimum – State of Wisconsin NP or PA licensure

If Nurse Practitioner:
Active RN and APNP license in State of Wisconsin AND
Active board certification as an NP in Adult; Adult/Gerontology Acute Care; Adult/Gerontology Primary Care; Emergency, Family, or Gerontology

If Physician Assistant:
Active PA license in State of Wisconsin AND
Active PA board certification

All Nurse Practitioner and Physician Assistant applicants must have:
Active DEA license or ability to apply for such license prior to or upon hire
CPR certification and advanced life support certifications if relevant to practice area (ACLS, PALS, NRP)

Physical Requirements:
Medium: Ability to lift up to 50 pounds maximum with frequent lifting/and or carrying objects weighing up to 25 pounds.

Work Schedule:
Monday through Friday from 8:00 am to 5:00 pm with some early or late meetings as needed.
